DESCRIPTION STATEMENT TO PROPOSED CLIENT
INTRODUCTION AGENT
SECTION A. (1)
The Introduction Agent is A Select Choice Pty Ltd A.C.N. 085221987 of Suite 32 Stanhill Building 34 Queens Road Melbourne 3004 Telephone Overseas 613 98674000 Australia 03 98674000. The introduction agent and it's staff will select introductions for clients in accordance with the answers stipulated on their own personal profile and will endeavour to provide introductions through it own membership which meet the requirements of the client. The main aim being for the client to form friendships with the view to a lasting relationship. In cases where membership permits the Introduction Agent may place advertising on behalf of the client and pass on introductions to the client when suitable people are found.
DESCRIPTION AND COST OF EACH SERVICE, METHOD OF INTRODUCTION
SECTION B.
LISTING SERVICE Listing service is available to people at the discretion of the Introduction Agent or it's representative and the following conditions apply.
(1) The client is deemed to be an acceptable person for this service and the client meets all other criteria set out in this section and sections C. titled: Proposed Clients Obligations.
(2) The client will be enrolled at $60.00 + $6.00 GST for one year if they come to our main office and fill in a profile in the presence of the introduction agent or it's staff. Where the client cannot attend the office of the introduction agent but joins through a consultant at the consultant's office or their own home or any other place a fee of $25.00 + $2.50 GST on application will apply to this membership to cover consultants or the Introduction Agent's out of pocket expense. This fee will be in addition to the $60.00 + $6.00 listing fee. Note ( This fee may be wavered in some cases of special promotion or at the discretion of the Introduction Agent or his/her representative with the permission of the Introduction Agent.)
(3) The client will be entitled to receive introductions only when a client in a paying membership selects them as a most suitable person. The client cannot request introductions but will receive introductions as other clients select them as a most suitable match. There is no claim that any introductions are promised.
(4) The client will be deemed to be a member from the date of signature of the agreement providing all other criteria are met, and they are found to be a suitable person to be accepted as a member. See Section C.
(5) The Method of introduction will be by mail or E-mail and a telephone contact number or mail or e-mail contact number will be supplied with the introduction form. Instructions for further introductions will be supplied with this form. Note: Under some levels of service clients may have more privileges. These privileges will be specified under that level of service in the description of membership for that level of service.
(6) The client signs and agrees to abide by the conditions of service document and all other commitments given in this document and any other commitments required by the client in relation to and specified in the Introduction Agency Act. A copy of Act No 75/1997 is available to the public and a copy may be purchased from the office of Information Victoria.

BASIC MEMBERSHIP Special Introductory offer: Price $200.00 + $20.00 GST Payable on Joining
Normal Price $300.00 + $30.00 GST
Method of payment: $90.00 + $30.00 GST Deposit on enrollment and 3 THREE Monthly payments of $70.00 1st payment due on the same day of the following 3RD Month Three month after the agreement is signed. Note: The G.S.T. goods and services tax amount will be forwarded to the appropriate government department.
Basic Service is for a period of twenty four months. Their is no limit to the amount of introductions under this membership other than time restraints because of the method in which procedures are carried out (see section A. (5).) The Introduction Agent will endeavor to provide a minimum of six introductions. The introductions will be posted to you in an unmarked envelope or forward by e-mail. Membership is renewable.
Note: Special introductory information. The introduction agent is appointing agents and franchisee's in Australia and other countries, the business A Select Choice Pty Ltd being a Victorian Government licensed agent in the State of Victoria Australia and is endeavoring to sign as many people as possible in order to satisfy the requirement of clients to give them a high percentage chance to find a suitable partner.
PRIORITY MEMBERSHIP Special introductory offer: $300.00 + $30.00 GST
Method of payment: $90.00 + $30.00 GST Deposit on enrollment and 3 THREE Monthly payments of $70.00 1st payment due on the same day of the following 3RD Month Three month after the agreement is signed. Note: The G.S.T. goods and services tax amount will be forwarded to the appropriate government department.
Normal price: $400.00 +40 GST
Method of payment: $120.00 + $40.00 GST Deposit on enrollment and 3 THREE Monthly payments of

Priority Service is for a period of twenty four months or until the client has been introduced to twelve compatible people. Their is no limit to the number of introductions under this membership other than time restraints because of the method in which procedures are carried out. See section A. (5). The Introduction Agent will endeavour to provide a minimum of twelve introductions. If necessary membership will be extended until twelve suitable matches has been achieved. Priority members will be given first preference when matching is done. The introductions will be posted to you in an unmarked envelope. Membership is renewable.

PERSONAL PRIORITY MEMBERSHIP Special Introductory offer: $800.00 + $80.00 GST

Normal Price $1000.00 + $100 GST
Method of payment: $300.00 + $100 GST Deposit on enrollment and 3 Three Monthly payments of $233.33 1st payment due on the same day of the following third month three months after the agreement is signed.

Personal Priority Membership
This membership is for a period of twenty four months and the client will be assigned a consultant either in the matching department or one who will liaise the matching department on behalf of the client. The consultant will be available by telephone to the client and can be contacted at any time in regard to introductions or updating his/her profile or further requests for introductions. The Introduction Agent will endeavour to provide a minimum of twelve introductions. If necessary membership will be extended until twelve suitable matches has been achieved. Personal priority members will be given preference and matching will be supervised by his / her personal consultant. The introductions will be posted to you in an unmarked envelope or forwarded by e-mail. At any time during the course of this membership the client can request to be put on hold. The period in which your membership is on hold will be added as an extension of the membership. The membership is renewable.
V.I.P SERVICE: Is available on request:
This service can be negotiated between the client and the Introduction Agent and the price for this service will be determined by the client's own personal requirements. Payment for this service will be by 30% of the total agreed price plus 10% GST and the balance to be paid in three equal three monthly instalments until the balance is paid in full.
SECTION C.
PROPOSED CLIENT OBLIGATIONS
(1.) The client must provide sufficient information via their personal profile and application form for the Introduction Agent to be able to perform the service as described and answer all questions regarding the client's own situation and choice. These answers must be honest in relation to themselves, and a good assessment in relation to the people they wish to meet.
(1.a ) Where a client's information attracts more than one person with the same compatibility. The client will be introduced to the person who has a higher percentage when the other personal criteria is checked in reverse manner with the clients.
(2.) You must keep the Introduction Agent informed at all times in relation to any changes to your circumstances which may affect the course or outcome of introductions, this includes any changes of address and contact arrangements.
(3.) You must make contact with any person you are introduced to. Thereafter you resume responsibility for the decision to meet. There is no requirement on behalf of the Introduction Agent that you meet. The decision is your own. Please note the contacts supplied will be counted as an introduction by the Introduction Agent and will be credited against time constraints in relation to the time between introductions.
(4.) Upon receiving an introduction you must at all time act in a proper and caring manner in relation to the other person an accept the other persons decision in relation to whether they wish to meet or continue to keep in contact.
(5.) You must abide by the terms and conditions of the Introduction Agent and keep the Introduction Agent informed of any matter which may affect the terms of this arrangement. You must provide two forms of positive identification to the Introduction Agent, also a recent photograph.
(6) You have the right to cancel the introduction agreement with in 3 days of signing the agreement. If you cancel the agreement within three days of signing it the Introduction Agent will be entitled to retain $50.00 or 10% of the total amount payable under this agreement which ever is the lesser amount.
(7) The client must agree to and abide by conditions in the Conditions of Service document provided separate to this document and signed by the client.

SECTION E.
COMPLAINTS PROCEDURES:
During the time of membership should and situation arise where a grievance or complaint arises the member should contact the Introduction Agent and outline the complaint and then confirm the situation by writing a letter to the Introduction Agent. On receipt of the letter the Introduction Agent will look into the matter and try to resolve the issue.
If after 28 days the matter is not resolved the Introduction agent and the client may agree upon an extension of time or may elect to settle the matter through a mediator at the Consumer Affairs Department.
If this cannot be agreed upon the client and the Introduction agent will be free to seek advice through their advisers or their legal representatives in respect to resolving the grievance
SECTION F.
EARLY ENDING OF AGREEMENT
You may wish to terminate the introduction agreement and may do so at any time before the period of the membership expires. If you chose to do so you will be obliged to notify the Introduction agent in writing. Unless otherwise agreed to by the Introduction Agent you shall be obliged to pay to the Introduction Agent the full amount outstanding as if the agreement had ran its full course.
SECTION G.
REFUND POLICY:
In accordance with the law and your rights under the law, no refund is allowed other than in relation to the Introduction Agency Act 1997 which states as follows:
31. Person may withdraw from agreement within 3 days without penalty
(1) A person who has entered into an introduction agreement may withdraw from the agreement at any time before the expiration of 3 clear business days after he or she receives a copy of the signed agreement.
(2) To do this, the person must within the time-
(A.) Give, or cause to be delivered to, the Introduction Agent; or
(B.) Leave at the address shown as the Agent's address in this agreement; or
(C.) Send by facsimile or by other mechanical means to the agent-
a written notice signed by the person that states that she or he withdraws from the agreement.
(3) If a person withdraws from an agreement under this section-
(A.) The Introduction Agent is entitled to $50.00 (or any other amount fixed by the regulations) or 10% of the total amount which is to be paid to her, him, or it under the agreement ( whichever is the lower amount); and
(B.) The agent must refund all moneys paid to her, him or it under the agreement by ( or on behalf of ) the person ( other than any amount the agent is entitled to under paragraph (A); and
(C.) The person is not liable to the agent in any way for withdrawing from
the agreement ( regardless of anything to the contrary in the agreement).
(4) An Introduction Agent who must refund money to a person under subsection
(3)(B) must do so within seven days of receiving the notice of withdrawal
(5) Section 26 overrides sub-section (3)(A) and (B).
FINAL SUMMARY
The Introduction Agent cannot guarantee that you will meet you ideal partner or that you will form a lasting relationship. This depends on your own decisions, frame of mind and attitudes to the introductions that you receive. The Introduction Agent can only offer advice and support in relation to these introductions. The Introduction Agent retains sole discretion in relation to all introductions. The Introduction Agent will be guided by your answers relating to introductions and will maintain the integrity and confidentiality in relation to the clients personal information and will only release sufficient information so that the client can make contact with the people they are being introduced to.
